DOC: add checklist to MR template

Summary

  • Add checklist to MR template (see below).
  • Change title levels to make headers smaller
  • Comment out implementation details sub titles

Rationale

Avoid forgetting important checks before merging. Easier to recall items of CONTRIBUTING.md
