Fix to compile CookBook

build_cookbook into master

Some parameter names in the applications SetDocExampleParameterValue were incorrect. I fixed them so that the CookBook can be built.

Also, I added a missing include in include/otbSARTopographicPhaseImageFilter.h because a header test was failing.
